Important information:
24 hour notice is required for cancellations. If cancelled with less than 24 hours notice, a $50 fee will be applied, which must be paid in full before another appointment can be booked.
Payment is due at time of service. Payment can be in cash or emt to chubby.cheeks.LAC@gmail.com. You will be provided with a digital receipt once payment has been received.
Before your visit, you will need to complete and return a “consent for treatment” form and “intake form.”
Initial at-home visit $225
Includes initial 60 minute consultation, oral assessment, observation of a feed, working towards following your feeding plan.
Additional 30 minute add-on $75
Additional 45 minute add-on $100
Text support for 10 days after the initial visit $50

Nipple shield support $75
30 minute visit will cover sizing, why and how to use, how to clean and when to wean off using.
Must be able to print a template used to find the right size for your body.

Pumping support $75
30 minute visit will cover sizing, how to pump, how to clean, pumping plan and how to wean off pumping.
